'Green' walls bad for office worker health: Study

In hot and polluted environments, indoor air
pollution may be worse with 'green' walls.
Going green is a growing trend - but in the case of living 'green' walls in offices, it might be a bad idea.

In fact, they could contribute to poorer air quality indoors, experts say.

Researchers of the University of York recently looked at the levels of ultrafine particles (UFPs) in hot and polluted environments. Such particles are a health concern as they can carry potentially toxic substances into the lungs.

The scientists simulated typical UFP levels in Athens, Helsinki and Milan offices during a heatwave and typical summer temperatures. The three cities were selected to compare contrasting climates and locations across Europe.

The researchers found that indoor concentrations of UFPs were highest in the Milan and Athens offices, reflecting high outdoor air pollution levels in these cities.

The pollutants make their way indoors through doors, windows and ventilation systems as well as through gaps in building materials.

However, indoor UFP concentrations were well above those expected through penetration of outdoor particles alone.

The researchers wanted to know why and realized they were a result of high concentrations of reactive volatile organic compounds (VOCs) outdoors, emitted by plants and trees.

These reactive VOCs include limonene, a naturally occurring compound emitted by plants and trees responsible for the citrusy smell in lemons and oranges, and pinene, emitted by pine trees.

Once in the atmosphere, such compounds rapidly oxidise to form a range of gas-phase and particle-phase products, which exist in a dynamic equilibrium depending on the conditions.

During heatwaves such as that experienced during 2003, emissions of VOCs increase in high temperatures and the formation of the secondary gas and particle-phase products becomes very efficient.

When outdoor air is drawn into an office air inlet, it is often filtered to partially remove outdoor particles. However, removing these particles disturbs the equilibrium of the secondary products and in order to re-establish a balance, new particles quickly form once the air reaches the office environment.

Therefore, indoor UFP concentrations are seen to be much higher if reactive VOCs exist outdoors near an office air inlet, as the impact of air filtration is lessened.

This finding is significant as, for the first time, indoor UFP formation is shown to be linked to the oxidation of outdoor plant and tree species in heatwave conditions.

Given the increasing popularity of green walls covered in plants and vegetation, their prevalence in hot, polluted locations could exacerbate indoor air pollution.

The filtration of air in modern office blocks is also seen to be less effective than expected, and this may explain why expected health benefits are often not realised when particle filters are added to a building.

Keeping workers safe should be a company's first priority

OSHA proposed a steep fine for repeat and
serious violations in worker safety guidelines.
When companies get lax with safety protocols, it can get costly.

United States workers are protected by gyidelines issued by the Department of Labor's Occupational Safety and Health Administration (OSHA). When OSHA receives complaints and starts investigating, companies better take note.

Fines for serious and repeat citations can run into the thousands.

In a recent case of an electroplating company in Connecticut, OSHA issued two repeat and 11 serious citations and proposed penalties of $48,304.

The company allegedly exposed workers to chemical and mechanical hazards, some of them were repeat violation from a previous investigation in 2010.

This time, OSHA found that the company failed to

  •     Determine employees’ initial exposure levels to lead and cadmium, two toxic substances in use at the workplace;
  •     Provide workers with training on cadmium hazards;
  •     Prevent cadmium buildup on machinery;
  •     Evaluate employees’ ability to safely operate forklifts;
  •     Ensure that employees who wear respirators are medically able to do so;
  •     Prevent employees from consuming food and drink in areas where the toxic substance hexavalent chromium was present;
  •     Separate flammable spray operations by at least three feet;
  •     Provide appropriate training to emergency coordinators and employees expected to fight fires; and
  •     Ground and shield an electric lamp against damage.

“Employees at this plant work with highly hazardous chemicals. It’s imperative that their employer take all necessary steps to protect their health and well-being at all times,” said Warren Simpson, OSHA’s area director in Hartford.

“That includes monitoring exposure levels, providing proper and effective protective clothing, and ensuring that employees are properly trained.”

Workplace safety apps on the rise

New apps help with occupational health and safety tasks.
As the world keeps changing, so does the world of occupational health and safety.

While technology continues to evolve, workplace safety needs to adapt as well.
That’s why cell-phone apps designed for occupational health and safety functions have become important in the marketplace.

New programs for smartphones and tablets can assist with all kinds of tasks, ranging from safety inspections and incident reports to emergency alerts and contacting remote colleagues when alone and in danger.

“Lives are at stake,” says Matthew Ross, media manager with ProntoForms Corporation, a mobile-solution company based in Kanata, Ontario. “People absolutely need to be able to process these types of info as fast as possible.”

ProntoForms has created an app that sends and receives forms and vital work information quickly.

“With the push of a button, the info is sent to wherever you like,” Ross explains. “You can send to a variety of cloud services.”

Such forms could include inspection checklists, safety lists, data on hazardous materials, action reports and even statements from accident witnesses, he adds.

“We’ve got such incredible positive feedback from clients because the processing is so much faster.”

Apart from increased safety, a side benefit of the ProntoForms app is a sharp reduction in paperwork.

“People are looking to help make their lives easier and make their jobs easier,” says Jason Grouette, business manager of the personal safety division for 3M Canada in London, Ontario.

3M Canada provides a practical new app that assists employees assigned to buy safety equipment for their companies, including distributors, health and safety managers and some end users.

Simply called Safety, the app gives the user instant access to more than 2,400 workplace safety products available from 3M.

Honeywell Safety Products in Morristown, New Jersey offers a similar product, the Media App, which gives users access to product information and learning resources on personal protective equipment available from Honeywell.

So why have workplace safety apps become more prevalent?

“We are in a different world that we were a decade ago,” says Grouette. “People have an expectation of finding answers very quickly and addressing problems quickly.”

“It’s driven by a bunch of things,” explains Ross. “The technology on Smartphones and tablets has gotten stronger and more powerful. So it has enabled us to include better features, time-savings, cost savings, high-productivity features. Another thing is the BYOD trend – bring-your-own-device trend – in businesses, so companies are more comfortable with employees using their own devices.”

Ross points out that the construction and oil and gas industries tend to supply ProntoForms’ biggest customers for health and safety products. But restaurant chains also use the ProntoForms app for information about cleanliness and other oh&s issues, he adds.

“The speed of data collection and processing is something that is invaluable in the industry.”

Grouette lists oil and gas, mining and manufacturing as sectors that benefit highly from 3M’s products, including the Safety app.

Many safety apps, including some of the aforementioned ones, are available to employers and workers for free.

Welders exposed to toxic substances at storage tank manufacturing company

Welding without proper safety precautions can expose workers
to hazardous chemicals and fumes. 
Workers welding stainless steel and other alloy steels containing chromium metal at a Wisconsin bulk storage tank manufacturer were exposed to hazardous levels of hexavalent chromium.

At high levels, hexavalent chromium can cause lung cancer and respiratory, eye and skin damage.

After a complaint, U.S. Department of Labor Occupational Safety and Health Administration inspectors visited Imperial Industries in Rothschild and identified two willful and 12 serious safety violations.

Proposed penalties total $161,100.

"Each year 50,000 workers die from exposures to hazardous substances like chromium during their careers. Failing to take steps to limit exposure to this dangerous substance is inexcusable," said Robert Bonack, area director of OSHA's Appleton office.

"Workers pay the price when companies don't follow standards to reduce injuries and illnesses. Imperial Industries needs to take immediate steps to comply with safety and health standards."

Inspectors determined employees were exposed to hexavalent chromium at levels exceeding permissible exposure limits while welding steels containing chromium metal. Chromium is added to harden alloy steel and help it resist corrosion.

Additionally, the company failed to implement engineering controls to reduce and monitor exposure levels among workers.

The November 2014 investigation also found workers endangered by amputation and struck-by hazards because machines lacked safety mechanisms. Numerous electrical safety hazards were also identified, and workers were found operating damaged powered industrial vehicles.

Imperial Industries manufactures heavy gauge metal industrial tanks that are typically mounted to commercial trucks.

Under the Occupational Safety and Health Act of 1970, employers are responsible for providing safe and healthful workplaces for their employees.

Hotel cosmetics manufacturer exposed employees to dangerous chemicals

OSHA found new and repeated hazards
at the manufacturing facility.
SYRACUSE, N.Y. – A leading maker of soap and shampoo for hotels and retail sale exposed workers to chemical and fire hazards and blocked emergency exit routes, the U.S. Department of Labor's Occupational Safety and Health Administration found in a November 2014 inspection.

A company that provided it the manufacturer with temporary employees was cited for chemical-related hazards.

The fact that new and repeated hazards were found shows Marietta Corp. must take worker health and well-being more seriously.

Acting on employee complaints, inspectors visited Marietta Corp. in Cortland and found flammable liquids were not stored or used properly; employees with respirators were not trained or checked medically; and containers with hazardous chemicals were not labeled correctly.

Similar hazards were found at the Cortland plant in 2011 and at the company's Chicago facility in 2010.

In addition, inspectors found workers without needed eye and face protection and emergency eyewash stations; employees not trained in the use of dangerous chemicals; and forklift operators who did not receive refresher training.

In total, Marietta was cited for three repeat and six serious violations with $103,800 in fines.

Select Staffing, the temporary agency that recruits workers for Marietta, received two serious violations with $10,000 in fines proposed. The staffing agency did not provide workers exposed to dangerous chemicals with proper eye and face protection; lacked accessible data sheets for hazardous chemicals; and did not prove that a hazard assessment had been done to determine what protective equipment employees would need.

In April 2013, OSHA announced an initiative to improve workplace safety and health for temporary workers, who are at increased risk of work-related injury and illness. The initiative includes outreach, training and enforcement to ensure that temporary workers are protected in their workplaces.

OSHA and the National Institute for Occupational Safety and Health have issued a "Recommended Practices*" publication that focuses on ensuring that temporary workers receive the same training and protection that existing workers receive.

The companies have 15 business days from receipt of their citations and proposed penalties to comply, meet with OSHA's area director, or contest the findings before the independent Occupational Safety and Health Review Commission.

Toxic solvents underneath IBM building defy cleanup

Buildings on contaminated soil may
expose workers to harmful chemicals
through soil vapor intrusion: Experts.
ENDICOTT – After 35 years, IBM Corp. contractors have stanched the flow of industrial solvents into a commercial and residential district in the heart of the village, but they have yet to find a solution for the source of the problem at the company's former flagship manufacturing plant.

Officials recently reported that efforts to intercept and remove the subterranean flow of hazardous chemicals coming from the industrial complex — now owned by Huron Real Estate Associates — have been successful.

Consequently, health risks to a nearby neighborhood have been eliminated.

Yet they have no remedy for a concentrated pool of solvents directly under the manufacturing site, where at least 1,500 people still work.

It may take years before a proven remedy is found, according to Alex Czuhanich, an engineering geologist with the state Department of Environmental Conservation, although the agency has no timetable for the project.

The pollution, discovered in 1979, includes trichloroethylene (TCE) and other solvents used as industrial degreasers that have been linked to maladies ranging from cancer to birth defects.

IBM used vast quantities of the solvents to manufacture printed circuit boards during the company's heyday from the 1950s through the 1970s.

The contaminated hot spot — known as "the source area" on DEC records — lies under an area the size of eight village blocks, and it was once bustling with the delivery, handling, storage, transport and liberal use of solvents.

The source area encompasses a railroad corridor where chemicals arrived in bulk, loading docks where they were handled, and a network of tanks and pipelines that stored and transported virgin chemicals and chemical waste to and from various manufacturing lines throughout the campus, according to DEC records.

Situated over and near the source area, and blocking access to cleanup, are dozens of buildings, most of them massive, bunker-like, cement structures built during the Cold War era and some well before.

The contaminated ground is dense with tunnels, boiler rooms, tank rooms, crawl spaces, cables, pipes and conduits.

"There is just so much infrastructure there," Czuhanich said. "Trying to get anything in there (to remedy the problem) is difficult, at best."

To date, nearly 70,000 gallons of solvent — more than 40 tons — have been pumped from the ground. Officials don't know how much is left, according to DEC spokesman Peter Constantakes.

Court records filed by attorneys representing residents suing IBM for damages from the pollution put the solvent pool up to 1 million gallons "that had apparently collected over many years from leaking pipes and tanks."

Reluctance by IBM and the DEC to publicly discuss the source area has added to uncertainty about its status. IBM spokesman Todd Martin did not reply to phone calls or written inquires on the subject.

Safety debated

IBM sold the plant in 2002 to Huron Real Estate Associates, which now rents space to BAE Systems Electronics, i3 Electronics (formerly Endicott Interconnect), Binghamton University and other smaller firms that collectively employ between 1,500 and 2,000 workers.

TCE, the main contaminant under the campus, is not commonly used in bulk by industry in the U.S. anymore, though it is a pervasive contaminant at many Cold War-era industrial and military sites throughout the country.

In addition to the former IBM site, notorious TCE legacy sites in the Southern Tier include the CAE Electronics site in Hillcrest, and the Morse Industrial site in Tompkins County, both linked to contamination of nearby residential neighborhoods.

The TCE problem is as stubborn as it is complex. TCE is one of a class of chemicals that tend to be heavier than water, so they sink through the water table and adhere stubbornly to soil particles.

They also give off toxic fumes that rise from the ground, ending up in basements, crawl spaces and — ultimately — circulating into living spaces through a process known as vapor intrusion.

There is no national standard to limit TCE exposure in air, although there has been a political push to develop one since the federal Environmental Protection Agency determined in 2011 that the chemical was a carcinogen and a "non-cancer health hazard."

Just how much exposure presents a risk remains a controversial topic. Levels that might not affect one person could make another seriously ill. Pregnant women are particularly vulnerable.

According to the latest EPA assessment — challenged by the chemical industry — risks for non-cancer illnesses, such as birth defects, from short-term TCE exposure in residential settings are found to increase statistically at levels at or above 2 micrograms per cubic meter.

The agency has set the threshold for risks from short-term exposure in an industrial setting at 7, based on the assumption that people spend less time at work than in their homes.

Cancer risks become discernible for chronic exposure — the type people might experience in homes — at levels beginning at 0.43 micrograms, according to an EPA assessment.

New York state has set a broad guideline of 5 for residential and commercial exposure. But the agency often requires action at levels below the threshold when feasible.

Feasibility remains the issue at the Huron campus. Indoor air samples at 42 buildings collected in 2005 — the last time the state oversaw testing — ranged from zero to 17 micrograms per cubic meter in some areas that tended to be occupied. Levels were much higher in other areas — often registering between 50 and 300 micrograms in tunnels and tank rooms below Building 18, for example.

Concentrations in the soil directly below the buildings often exceeded 10,000 and sometimes were over 100,000 micrograms.

In 2005, the state health department determined that the TCE levels at the Huron campus present a "low" health risk to people working there, according to a report at the time. That means state health officials "do not expect to be able to associate health effects" from exposure.

Health department spokesman Jeffrey Hammond said the guideline remains "under review" as more information comes to light "to ensure that previous decisions and recommendations continue to protect public health."

In 2005, a health department study found elevated rates of testicular and kidney cancers, and birth defects that were "statistically significant" in an area affected by solvent pollution in the area south of the former IBM plant, and several blocks to the southwest of the plant polluted by an undetermined source. (The area has since been cleaned by the IBM remediation efforts.)

The results of a study by NIOSH and the health department evaluating birth outcomes of women who worked at the plant is expected to be released later this year.

US salons need to nail occupational safety: Advocates

Nail salons will employ more than 100,000 workers by 2022

of reproductive age, experts say.
When New York City Public Advocate Letitia James, JD, announced that her office was releasing a report on nail salons last year, it was anything but a frivolous task.

The policy report, “How Safe is Your Nail Salon?,” released in September, took a look at health and safety practices for both consumers and workers in New York City’s nail salons.

And with more than 2,000 businesses licensed to do manicures and pedicures in the city alone, the health of a large swath of the public is affected.

In New York, the salons are regulated by the state — which has just 27 inspectors to help maintain their safety, James told The Nation’s Health.

The health and wellness of nail salon employees is no small matter, as the U.S. Bureau of Labor Statistics estimated there were 86,900 manicurists and pedicurists in the U.S. in 2012. That number is expected to rise to 100,400 by 2022.

But that estimate is probably far too low, according to the California Healthy Nail Salon Collaborative, which estimates there are 97,100 manicurists in California alone right now.

Up to 80 percent of those workers are Vietnamese immigrants, and more than 50 percent are women of reproductive age.

Duyen Tran, MPH, an APHA member and the interim outreach coordinator for the collaborative, says there are several reasons that nail salon work appeals to young women in the Vietnamese community.

Some of it is the flexibility working in a nail salon can afford: Employees can tailor their schedules around their families’ needs. Another reason is the ease with which a worker can enter into the industry and start making money. Training courses, which are 12 to 18 months long, and exams are offered in Vietnamese.

“To do nail salon work you don’t need high English proficiency,” Tran told The Nation’s Health. “It doesn’t require intensive English training, so it’s really an opportunity for this recent immigrant population to enter the workforce and use it to support their families and communities in a very short time.”

But joining the workforce means exposure to known dangerous products — and potentially unknown dangers, as well.

Three chemicals pose most risks to workers

The biggest risks to nail salon workers are “the toxic trio:” Toluene, formaldehyde and dibutyl phthalate are the most common and dangerous ingredients in nail products, including polish and polish remover, that have been linked to serious health risks.

According to the Centers for Disease Control and Prevention, toluene exposure has been linked to tiredness, confusion, weakness, drunken-type actions, memory loss, nausea, loss of appetite and hearing and color vision loss. High levels of exposure have been linked to kidney damage.

Formaldehyde exposure can lead to irritation of the eyes, nose and throat, causing tearing, and skin irritation, according to CDC, and is a known carcinogen. CDC’s Agency for Toxic Substances and Disease Registry notes that dibutyl phthalate is linked to organ development issues in fetuses when exposed during gestation.

The toxic trio can be transmitted as airborne particles, through product contact with skin or eyes and via unintentional transfer of the materials to uncovered food, drink or cigarettes, according to research from the California Healthy Nail Salon Collaborative.

The Occupational Safety and Health Administration has reported that chemical levels can exceed 826 parts per million during the application of acrylics in nail salons, but proper ventilation can drop that to 12.4 parts per million.

Despite these risks, in Nails Magazine’s 2014-15 report, “Nails Big Book: Everything You Need to Know About the Nail Industry,” 34 percent of nail salon workers reported that they never wear protective gloves while working.

Sixty-one percent said they never wear a mask while working. And more than half reported having work-related health concerns. Twenty-three percent said they were uninsured.

Salons can promote safety for workers

Though self-reported low numbers of nail salon workers take safety precautions, state and federal government regulations require certain steps to be taken to ensure worker safety.

OSHA distributes “Stay Healthy and Safe While Giving Manicures and Pedicures: A Guide for Nail Salon Workers,” which outlines workers’ rights to health and safety for both employees and salon owners.

The guide has been translated to Vietnamese, Spanish and Korean. And OSHA has been working to reach out to communities to make sure workers’ rights are well-known, said Mandy Edens, MSPH, director of OSHA’s directorate for technical support and emergency management.

Auto parts store cited for exposing workers to asbestos and mold

The auto parts store failed to protect worker health and
safety, OSHA says.
KANSAS CITY, Mo. – A worker alleging the existence of asbestos, mold and hygiene hazards led to an inspection of an Advance Auto Parts store in Kansas City, where the U.S. Department of Labor's Occupational Safety and Health Administration found one repeated and 10 serious safety and health violations with fines of $60,000.

"Exposure to asbestos is a dangerous workplace issue that can cause loss of lung function and cancer, among other serious health effects. When Advance Auto uses an older building with presumed asbestos-containing material, such as floor tiles, it has a responsibility to conduct periodic air monitoring and must post warning signs for workers," said Barbara Theriot, OSHA's area director in Kansas City.

"The company also has a responsibility to maintain the building in a sanitary and safe manner. OSHA found persistent flooding, which caused mold growth and created lower-level slip and fall hazards. This is unacceptable."

OSHA inspectors tested bulk samples of furnace room floor tiles and found they contained 3 percent chrysotile, a form of asbestos. Sample air monitoring did not detect asbestos fibers circulating in the heating and air conditioning system.

However, particles could become airborne from deteriorating tiles and persistent flooding, a consistent issue throughout the building.

Asbestos is a naturally occurring mineral fiber used in some building materials before its health dangers were discovered. Asbestos fibers are invisible and can be inhaled into the lungs unknowingly. Inhaled fibers can then become embedded in the lungs.

Inspectors also found electrical safety violations and blocked exit routes at the store, resulting in the 10 serious violations. An OSHA violation is serious if death or serious physical harm could result from a hazard an employer knew or should have known exists.

OSHA also noted a repeated violation for failing to provide inspectors with injury and illness logs.

Based in Roanoke, Virginia, Advance Auto Parts was previously cited for this violation in a Delaware, Ohio, store in 2010 and a Lakeland, Florida, store in 2011.

OSHA issues repeated violations if an employer was cited previously for the same or a similar violation within the last five years.

Advance Auto Parts has 15 business days from receipt of its citations and penalties to comply, request an informal conference with OSHA's area director, or contest the findings before the independent Occupational Safety and Health Review Commission.

Schools need to manage mold and moisture: EPA

As you prepare for spring’s warmer weather, consider taking some time to review your district’s policies and procedures related to mold and moisture control.

Schools should be proactive in their moisture control practices, which are the key to controlling indoor mold growth.

Molds are a major source of indoor allergens and can trigger asthma in sensitive individuals.

Even when spores are dead or unable to grow, mold can cause health effects, such as allergic reactions.

The types and severity of health effects associated with exposure to mold depend, in part, on the type of mold present and the extent of the occupants’ exposure and existing sensitivities or allergies.

Prompt and effective remediation of moisture problems is essential to minimize potential mold exposures and their potential health effects. These negative health effects can lead to increased absenteeism and reduced performance in both staff and students.

How should mold be safely cleaned?

It is essential to clean up mold as well as excess water or moisture, because moisture is the key to mold control.

If the excess water or moisture problem is not fixed, mold will most likely grow again, even if the area is completely cleaned.

Clean hard surfaces with water and detergent and dry them quickly and completely.

Absorbent materials such as ceiling tiles may have to be discarded if the mold cannot be completely removed.

To clean up mold safely and effectively, staff should wear waterproof gloves during clean up and should not touch mold or moldy items with bare hands.

Respiratory protection should also be used in most remediation situations to prevent inhalation exposure to mold.

10 tips to control mold and moisture in schools this spring

  1. Establish a mold prevention and remediation plan within your greater IAQ management program.
  2. View the IAQ Reference Guide on Mold and Moisture for tips on identifying and correcting common mold and moisture issues.
  3. Inspect all school buildings for signs of mold, moisture, leaks or spills, or evidence of past water damage.
  4. Prevent moisture condensation by increasing surface temperature, installing proper insulation and improving air circulation.
  5. Eliminate sources of moisture by reducing indoor humidity; maintain indoor humidity levels between 30 and 60 percent.
  6. Respond promptly when you see signs of moisture and/or mold or when leaks or spills occur. Dry all wet areas within 24 to 48 hours.
  7. Perform regular heating, ventilation, and air conditioning (HVAC) inspections and maintenance as scheduled. Ensure HVAC drip pans are clean, unobstructed and flowing properly.

Welders at risk of toxic and explosive fumes

Blast kills temporary worker, critically injures another

Employers need to verify fire and explosion hazards: OSHA
MOSS POINT, Miss. — Two temporary workers hired to cut and weld pipes at the Omega Protein plant in Moss Point on July 28, 2014, had no idea and had no training to know that the storage tank beneath them contained explosive methane and hydrogen sulfide gases.

One of the two men found out later as he lay in a hospital with a fractured skull, internal injuries and broken bones. The second, a 25-year-old man named Jerry Taylor, died when the tank exploded.

The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) investigated the incident and has found four companies violated safety regulations that could have prevented the tragedy.

The companies are Accu-Fab & Construction Inc., Omega Protein, and JP Williams Machine & Fabrication, all in Moss Point, and Global Employment, in Pascagoula.

Accu-Fab, a metal fabricator, was contracted by Omega Protein to manufacture and erect a wastewater storage tank that required modification of existing pipes.

A staffing agency, Global Employment Services, provided Accu-Fab with the employees needed at Omega. JP Williams Machine, which provides industrial service and repair, was on-site the day of the explosion performing unrelated maintenance activities.

"The Omega Protein plant explosion shines a spotlight on how critical it is for employers to verify, isolate and remove fire and explosion hazards in employee work areas," said Eugene Stewart, OSHA's area director in Jackson. "If the employer ensured a safe environment, this tragic incident could have been prevented."

Repeated violations

OSHA issued 13 citations to Omega Protein, a producer of omega-3 fish oil and specialty fish meal products, for willful, repeated and serious safety violations.

OSHA issued a willful citation for exposing employees to fire and explosion hazards due to Omega management's failure to inform Accu-Fab that the storage tank contained wastewater that could generate hydrogen sulfide and methane gases, which can be highly explosive and toxic, even at low concentrations.

The repeated violations involve not having standard railings on open-sided floors and platforms and failing to label electrical boxes properly.

Omega Protein was cited previously for these same violations in 2012. Additionally, the serious hazards included allowing workers to weld and cut piping on an improperly prepared storage tank containing explosive methane and hydrogen sulfide gases and failing to label or tag the storage tank to note that it contained hazardous chemicals.

OSHA cited Accu-Fab for one willful, four serious and two other-than-serious violations. The willful violation was issued for failure to train workers on chemical hazards in the work area, such as hydrogen sulfide, methane, welding gas and paints.

Global Employment Services was issued a serious citation for this same hazard.

Additionally, both employers were cited for a serious violation for failure to instruct employees about avoiding unsafe work conditions.

Accu-Fab was also cited for failure to ensure employees working on top of a storage tank at heights of up to 29 feet were wearing fall protection and for not recording this fatality or two other recordable injuries.

JP Williams was issued one serious citation for improperly storing oxygen and acetylene cylinders.

Exposure to acetylene can cause headache, dizziness, asphyxiation and even frostbite.

Proposed penalties for the four companies total $187,620.

OSHA has conducted 13 inspections at Omega Protein facilities in Mississippi, Virginia and Louisiana since 1998. The company received citations for noise exposure, personnel protective equipment, machine guarding, welding and cutting and electrical hazards.

Accu-Fab has three prior OSHA inspections, most recently in 2002, and has been cited for scaffolding, forklift, welding, cutting and electrical violations. Both JP Williams and Global Employment have no prior OSHA inspection history.

10 jobs that are bad for your lungs - Part 2

Knowing the dangers in certain occupations may help decrease the risk factors. The first five jobs that pose a threat to the lungs were:

1. Bartending and waitressing
2. Housekeeping and cleaning

Construction workers are often exposed

Workers who demolish old buildings or do remodeling can be exposed to asbestos used as insulation around pipes or in floor tiles.

Even minimal exposure to its microscopic fibers has been linked to a variety of problems. One is mesothelioma, a form of cancer, Von Essen says.

Exposure also seems to raise the risk of small-cell lung cancer and can lead to asbestosis, or scarring of the lung. Removal should be left to trained and licensed crews.

"Know where the asbestos is," Von Essen says. "Follow all the rules and don't take chances."

7. Farming

Working with crops and animals can lead to several disorders. Hypersensitivity pneumonitis is a rare but serious problem caused by repeated exposures to mold-contaminated grain or hay. The lung's air sacs become inflamed and may develop scar tissue.

Grain in metal bins can get moldy. Breathing dust from this grain can lead to fevers, chills, and a flu-like illness called "organic dust toxic syndrome." Farmers also are more likely to report a cough and chest tightness.

"We think about 30% of farmers who grow crops in this way have had that at some point," Von Essen says. Workers in hog and chicken barns sometimes get an asthma-like syndrome.

"Dust and ammonia levels together seem to be risk factors," she says. Keep grain from getting damp, ensure adequate ventilation, and wear a respirator.

8. Auto Body Spray Painting

People who work in auto body shops are often exposed to chemicals known as isocyanates. They're a significant cause of occupational asthma.

"It's frequently a career-ending disease where they need to leave their profession," Harber says.

The right protective equipment may
alleviate some of the risks.
Using quality respirators that are appropriate for your task can lessen the risk. It also helps to enclose the area being sprayed and to have a ventilated exhaust system. Better yet, replace hazardous materials with safer ones.

9. Firefighting

People who battle blazes are exposed not only to the fire, but also to other materials, including burning plastics and chemicals. Firefighters can significantly lower their risk of lung disease and other problems by using a "self-contained breathing apparatus" (SCBA). These devices should also be used during "mop up" or the clean-up period.

"Many of the chemicals are still in the air," Harber says. Ventilation also is critical.

10. Coal Mining

Underground miners are at risk for everything from bronchitis to pneumoconiosis, or "black lung." It's a chronic condition caused by inhaling coal dust that becomes embedded in the lungs, causing them to harden and make breathing very hard.

"This can cause progressive massive fibrosis and can kill people," Von Essen says.

Again, protective equipment can limit the amount of dust inhaled.

Combustible dust to blame for Ontario plant explosion

Companies have to make sure to protect
workers from combustible dust and more.
An explosion and subsequent fire at an industrial facility in Sarnia, Ont. left several employees hospitalized on Oct. 25. One of the workers died of his injuries two days later.

The incident took place at a plant belonging to Veolia Environmental Services, an international company that uses propane and oxygen to conduct thermal spraying of aluminum.

According to information from the provincial Ministry of Labour (MOL), a dust collection system outside of the building exploded and caused structural damage in the middle of the afternoon.

“Five workers were injured in the incident, including two who were critically injured,” confirmed MOL media representative Bruce Skeaff.

“Emergency services were dispatched and attended the scene. The five injured workers were transported to hospital.”

One of the critically injured parties was subsequently airlifted from Sarnia’s Bluewater Health hospital to a London hospital for further treatment.

Eight workers in total were inside the building at the time of the blast, according to media reports.

Skeaff added that MOL inspectors, firefighters and officials from the Ontario Fire Marshal’s office had also attended the scene of the explosion.

“A City of Sarnia engineer attended the scene and declared the building unsafe to enter,” he said.

The MOL issued a requirement to Veolia not to disturb the scene of the incident, under the Occupational Health and Safety Act. The fire was extinguished the next day, and a forensics investigator examined the scene as well.

Carol Gravelle, public relations officer with the Office of the Fire Marshal and Emergency Management (OFM), told COHSN that the office had seized evidence at the site as exhibits for testing offsite.

The OFM “worked with the Ministry of Labour, the coroner’s office, local police and local fire” to investigate the origins of the explosion, Gravelle said.

On Oct. 27, Const. Les Jones of the Sarnia Police Service (SPS) announced in a press release that one critically injured employee — the one who had been airlifted — had died earlier that morning. “Sarnia Police Service will not be releasing his name,” Const. Jones added.

A media statement from Veolia, issued on Oct. 26, said that the company was cooperating fully with the MOL and other authorities in their investigations.

“The employees of Veolia are deeply concerned for our co-workers,” the statement read, “and our thoughts and prayers are with them at this time.”

The MOL continues to investigate, as do the SPS and offsite investigators with the OFM.

OSHA cites Illinois garage for 5 repeat safety violations

Auto body shop workers may be exposed to dangerous
chemicals and other hazards, experts say.
A complaint investigation found that Transport Tech LLC failed to provide employees with an effective training program, including information on appropriate handling and safe use of hazardous chemicals at its Hillside repair facility.

The U.S. Department of Labor's Occupational Safety and Health Administration has cited the company for five repeat safety violations, carrying proposed penalties of $66,400.

"Workers have the right to know what chemicals they are exposed to and how to protect themselves against exposure, which can have severe health effects," said Angeline Loftus, OSHA's area director at the Chicago North office in Des Plaines.

"Employers have a responsibility to provide accurate information about the hazards their workers face each day, and Transport Tech failed to do that."

OSHA initiated an inspection on March 28, 2014, after it received a complaint alleging hazards at the shop, which provides repair services for trucks operated by the national carrier Central Transport LLC.

Transport Tech failed to put identification and warning labels on containers filled with hazardous chemicals.

The company also failed to have an eyewash station readily accessible and provide portable fire extinguisher training, as required.

In addition, floors at the facility were not kept clean and dry to prevent slips and falls. The company was cited for similar violations in 2011 at the same facility.

OSHA issues repeat violations if an employer previously was cited for the same or a similar violation of any standard, regulation, rule or order at any other facility in federal enforcement states within the last five years.

Transport Tech, based in Warren, Michigan, has 15 business days from receipt of its citations and penalties to comply, request an informal conference with OSHA's area director, or contest the findings before the independent Occupational Safety and Health Review Commission.

Warehouse workers tested for PCB exposure

workers sent home until further notice.
A Walmart returns processing center in Indianapolis is contaminated with a toxic substance, and hundreds of workers at the evacuated facility are now undergoing medical testing to see if they were exposed.

The contamination involves a massive warehouse, where logistics company Exel processes merchandise returned from Walmart retail stores.

The warehouse now sits empty after Exel ordered nearly 600 full-time and contract workers to evacuate the processing center on August 20.

On that day, supervisors met with employees at 3:45pm to announce the facility was shutting down immediately.

During the meeting, employees were not told the reason for the shut-down, only that they would continue to receive their normal pay and benefits and would not return to work until further notice, according to a longtime worker who asked not to be identified.

Five days later, Exel managers again met with employees at a nearby hotel to explain Walmart discovered the presence of a strange substance within the facility.

Testing showed the substance to be PCBs or polychlorinated biphenyl, a synthetic organic chemical compound that is highly toxic and classified by the US Environmental Protection Agency as "probable human carcinogens." The EPA says studies in animals provide conclusive evidence that PCBs cause cancer.

Over the past two weeks, Exel employees have been reporting to an east-side medical laboratory for blood tests, which Exel hopes will shed light on which employees were exposed to the PCBs and what impact - if any - the exposure might have on their health.

"It's a situation that continues to evolve, and we're working diligently with Walmart to understand it more," said Exel Vice President of Communications Lynn Anderson.

"We took an overly cautious role and decided we wanted to get out of the building right away. We are really trying to understand the extent of the contamination and the exposure and what it means for the future and the facility."

A Walmart company spokesperson says that Walmart made a joint decision with Exel to close operations "out of an abundance of caution."

"Walmart immediately hired an environmental consulting firm after a contractor servicing a return center we lease discovered the presence of PCBs, or polychlorinated biphenyls. Additional testing confirmed PCBs were present in the building, which is operated by a contractor, Exel Inc. We made a joint decision with Exel to close the facility out of an abundance of caution.

"Both the U.S. Environmental Protection Agency and the Indiana Department of Environmental Management (IDEM) have been informed and are investigating this matter. We are cooperating with the investigation, and early indications suggest that the contaminant is in the building materials.

"We have made arrangements for returned products from our stores to be sent to other return centers."

Unusual particles discovered

Anderson says the contamination was discovered by accident, while equipment was being moved inside the plant. That's when workers found an unusual residue and "particles that didn't look right."

Walmart hired a third-party company to test the residue, and according to Exel, the testing revealed the presence of PCBs.

How much PCBs and where did they come from? Exel and its employees are still looking for answers.

Exel plans to begin its own independent testing at the abandoned warehouse this week. In the meantime, it is actively looking for another facility to resume its operations. Exel has not ruled out the possibility of returning to the contaminated facility, but says that is unlikely - at least in the short-term.

Since the evacuation, Exel has hosted two face-to-face meetings with affected employees to provide them with information, and another meeting is scheduled for early October.

At the last meeting, workers were encouraged to take advantage of free blood tests.

PCBs are considered very dangerous to human health, and they are very hard to destroy. Banned in the United States for decades, they were commonly used as coolants and stabilizers in products such as fluorescent light ballasts, transformers, paints, cements, electrical components, pesticides, lubricating oils and sealants.

A known carcinogen, PCBs are linked to other serious health concerns including negative impacts on the immune, reproductive and neurological systems.

Chemical plant inundated with complaints from nearby residents

Residents close to a chemical plant
have been getting sick, they say.
PASCAGOULA – An Environmental Protection Agency inspector, during a tour of Mississippi Phosphates in 2009, walked through a puddle so acidic it ate leather off the inspector’s steel-toed boots.

The inspector’s vivid account is one of dozens from an inspection that year that yielded a biting administrative order from the EPA to the company, using the words “imminent and substantial” danger when referring to the possible impact on human health or the environment.

The federal agency ordered the company to fix uncontrolled leaks and spills of sulfuric acid on its grounds, and stop untreated discharges to the adjacent bayou and uncontrolled spills and leaks of phosphoric acid to unlined ditches.

The agency found more issues in 2011, and in 2012, reinforced its orders to clean up.

Then last year, the state Department of Environmental Quality shut down two plants at the fertilizer manufacturer because they were creating an acid mist that caused neighboring industry to evacuate workers.

All the while, a neighborhood less than a mile away says it was unaware of the reports. Cherokee Forest has 132 homes along heavily industrialized Bayou Casotte in east Pascagoula, where several industries contribute heavily to air emissions.

Residents have been complaining in recent years about thick dust, strong acrid smells and health problems.

They’ve told regulators what’s in the air and what’s on the ground at neighboring industry is coming into their yards, onto their cars, onto their skin and into their lungs.

The company says it has fixed its problems from the 2009 inspection.

“EPA conducted a final compliance inspection in October 2010 and confirmed that MPC had implemented all the corrective action items addressed in the 2009 order,” it said in a statement to the Sun Herald.

Over the last five years, MPC has continued to invest in its operation and environmental programs and work with the EPA and Mississippi Department of Environmental Quality to ensure future environmental compliance at the facility.”

So what’s the significance of the EPA inspector’s boot?

Howard Page, a community coordinator with the STEPS Coalition in Biloxi, who helped organize the neighborhood into a group, said although Mississippi Phosphates has made improvements, he believes the story of the acid puddle is relevant today.

“It shows how very real the threat is,” Page said. “It’s not the most recent event; things the company has been cited for and had to correct. But it captures the issue — how strong that acid is.

“If it does that to a boot, just imagine what it does when you’re breathing it as a mist. It’s a concrete example of what people are facing, not just odors or a possible concern.”

Mississippi Phosphates said it pledged more than $2.5 million to cleanup what the EPA found in 2009, has made other improvements and more recently has offered air monitors in the neighborhood to detect sulfuric acid.

But Page said he believes the chronic history of problems, “not one or two times, but a long series of incidents” presents a health threat to workers and nearby residents.

He said there’s a need to control the risk to workers and neighbors.

“The process they operate there that involves acid is a potentially dangerous process if it’s not managed and overseen well,” he said.

Page said one issue for the neighborhood has been substantiating their complaints. And though other industries are putting dust and chemicals in the air, remarks of trained EPA inspectors at Mississippi Phosphates in the 2009 report sound familiar to residents:
  • “the sampling team tasted a metallic flavor in the air that permeated to the back of the throat. Sampling team members complained of burning, stinging and itchy skin and eyes. The metallic taste remained with EPA personnel for at least two days after leaving the site.”
  • “The ongoing inhalation of sulfuric acid mist may pose substantial harm to personnel at and visitors to the facility.”
  • “The effects of discharges . extend far beyond the initial discharge zone, reaching to the other side of Bayou Casotte, and far into the commercial turning basin (of the bayou).”
  • “Low pH liquids facilitate the mobilization of metals such as arsenic, cadmium and chromium. Metals are readily leached from the soils and or sediment into groundwater upon contact with a solution of pH less than 3.5.” The puddle one inspector stepped in had a pH of less than 2 (the company’s permit allows a range of 6 to 9).
Today, the residents, complaining of health issues, find it is up to them to chase complaints and prove their case. And Page pointed out local government, the Department of Environmental Quality and the industries are also at the table looking for solutions.

Gas workers may be exposed to dangerous levels of benzene: Study

A new study this month reveals unconventional oil and natural gas workers could be exposed to dangerous levels of benzene, putting them at a higher risk for blood cancers like leukemia.

Benzene is a known carcinogen that is present in fracking flowback water. It’s also found in gasoline, cigarette smoke and in chemical manufacturing.

As a known carcinogen, benzene exposures in the workplace are limited by federal regulations under OSHA. But some oil and gas production activities are exempt from those standards.

The National Institute of Occupational Health and Safety (NIOSH) worked with industry to measure chemical exposures of workers who monitor flowback fluid at well sites in Colorado and Wyoming.

A summary of the peer-reviewed article was published online this month on a CDC website. In several cases benzene exposures were found to be above safe levels.

The study is unusual in that it did not simply rely on air samples. The researchers also took urine samples from workers, linking the exposure to absorption of the toxin in their bodies. One of the limits of the study includes the small sample size, only six sites in two states.

Dr. Bernard Goldstein from the University of Pittsburgh School of Public Health says the study is the first of its kind. Goldstein did not contribute to the study’s research, but he has conducted his own research on benzene. And he’s treated patients exposed to the carcinogen.

“These workers are at higher risk for leukemia,” said Goldstein. “The longer, the more frequently they do this, the more likely they are to get leukemia particularly if the levels are high.”

The study looked at workers who use a gauge to measure the amount of flowback water that returns after a frack job is initiated. A spokeswoman for NIOSH says none of their studies draw any conclusions about exposures to nearby residents, but focus specifically on workers.

But Dr. Goldstein says it shows that there could be potential risks to residents as well.

“We’re not acting in a way to protect the public who are at high risk,” said Goldstein. “And we can’t even tell you who is at high risk. Yet we’re rushing ahead in a situation where all of the data are telling us that there are risks.”

Authors of the NIOSH benzene study said that more research with larger sample sizes should be done, especially since there was so much variation in the levels observed at different times and well sites.

The researchers also listed a number of recommendations for industry to take to reduce benzene levels on the job site. These include changing tank gauging procedures, training workers, limiting exposure times, carrying gas monitors, using respiratory and hand protection, and monitoring exposure levels.

Sawmill ordered to pay for fatal fire

WorkSafeBC orders mill to pay $724,000 in penalties, levies for 2012 fire

The company that owns the northern British Columbia sawmill where two workers were killed and 22 others injured in a explosion and fire has been ordered by WorkSafeBC to pay more than $724,000 in penalties and levies.

The April 23, 2012 blaze at Prince George's Lakeland Mills Ltd. claimed the lives of Alan Little, 43 and Glen Roche, 46 and followed a similar deadly explosion only months earlier at the Babine Forest Products sawmill in Burns Lake, B.C.

The province's Criminal Justice Branch announced earlier this year it would not lay charges against either of the companies in the mill blasts because it feared the evidence collected wouldn't be admissible in court.

But WorkSafeBC said recently that Lakeland Mills breached the Workers Compensation Act and Occupational Health and Safety Regulations and ordered it to pay a $97,500 administrative penalty and a $626,663 claims-cost levy for violating the act and the regulations.

"The dollar value of a penalty or claims cost levy does not and cannot reflect the loss of lives and the pain and suffering of workers and families,'' the agency states on its website, adding the company has the right to appeal and review the penalties.

The order follows a similar $1-million ruling in April by WorkSafeBC against Hampton Affiliates, the owners of the Babine mill.

Lakeland Mills president Greg Stewart responded to Tuesday's report with a written statement, saying the company had just been notified earlier in the day.

"It will take some time to review the information,'' he said. "Only then will we be in a position to respond to WorkSafeBC's allegations.''

Shane Simpson, the New Democratic labor critic, called the penalties and assessments a slap on the wrist and of cold comfort to the families of the workers killed and the survivors.

He reiterated calls for an independent inquiry.

"We haven't got to the bottom of this,'' he said. "We haven't found closure for the families, and I'm afraid that's not going to happen until we have the kind of independent inquiry that has been called for across the board by most people other than the government.''

The fire at the mill broke out at about 9:30 p.m. on April 23, 2012, and WorkSafeBC found the mill's northeast corner exploded outward. A few seconds later another section, known as the bag house, erupted in flames, it found.

The explosion traveled east to west through the mill's operating level, destroying the mill, killing and injuring the workers.

All the evidence indicated wood dust was dispersed throughout the mill and in a high-enough concentration to explode, stated a WorkSafeBC report issued in May.

The report noted the primary explosion occurred an area of about three-square meters which was surrounded by a conveyor, steel-plated ceiling and exterior wall.

The report described the important relationship between containment and a fuel-like wood dust.

"If these components are contained and ignition occurs, the pressure develops to a degree that typically is violent and destructive,'' it stated.

The friction that ignited the blaze was caused when a piece of equipment known as a gear-reducer cooling fan failed, and a rotating shaft generated friction, heat and a temperature of 577 degrees Celsius, the report added.

As a result, the airborne dust burned away in the containment zone during the primary explosion and the secondary explosions, and fire leveled the mill, it stated.

The report also cited several underlying factors.

There was a lack of a dust-collection system and ineffective dust-control measures, as well as ineffective maintenance and inspection of the gear reducers, it found.

The configuration of the waste conveyor increased airborne wood dust as well, and wood and weather conditions played a role, the report noted.

"The weather conditions resulted in a very dry environment with low humidity,'' it stated. "The condition was compounded by the very dry beetle-killed wood. The dusts produced were drier, finer and migrated throughout the mill.''

Finally, the report cited "inadequate supervision of clean-up and maintenance staff.''

The agency said it has ordered every B.C. sawmill to assess the risks and hazards of combustible dust and implement effective dust-control programs.

Followup inspections have been ordered by WorkSafeBC at other sawmills and wood-processing operations, and the agency has ordered hazard alerts for gear reducers and wintertime conditions when there are increased risks.
